Documentation for Aadhaar Card: Seva Kendra Head clarifies

Sir,

It is seen that many people are approaching Aadhaar Seva Kendra (ASK) in Vijayanagar with invalid documents. This causes them unnecessary hardship and inconvenience.   Certain unscrupulous people are taking money and providing invalid certificates to uneducated or unwary persons. Hence this clarification in order to ensure that citizens are not taken advantage of and to avoid inconvenience:

The list of valid supporting documents is available at uidai.gov.in>My Aadhaar>Downloads>List of supporting documents. Standard UIDAI Format for certificate is available at uidai.gov.in>My Aadhaar> Downloads>List of supporting documents (Page 3).   In this certificate, photo must have signature and seal  of the Certifier. While signing on the photo please use marker  pen. Sign and seal must be partly on photo and partly on the certificate and must be clearly visible.

Certain important points to remember in the documentation for Aadhaar Card are:

POI — Proof of ID

For name correction

• Any one of the 32 documents mentioned in the list of supporting documents.

• Only Photo ID card is accepted for POI. • Documents used for POI should have clear photo of the resident.

• Name mentioned in the document and the name the resident requested for in his Aadhaar Card should be the same. • Complete name change is possible only with Gazette Notification.

• Marriage certificate (from Sub-Registrar Office) is accepted for name correction only if it contains the photo and name of the resident.

• Principal of the School can certify only the students who are currently studying in their school.

• School ID card should be of current session (2019 to 2020).

• Certificate or letter issued by the authority must and should have exact information. The validity of this certificate is 3 months from the date of issue.

•  Name correction can be done only TWICE in your lifetime. 

DOB (Date of Birth)

 For DOB correction

• Any one of the 15 documents mentioned in the list of supporting documents. • Name mentioned in the proof document and Aadhaar should be same.

• Certificate or letter issued by the authority must and should have exact information. The validity of this certificate is 3 months from the date of issue.

• Certificate for DOB proof can only be issued by a Group ‘A’ Gazetted Officer.

• DOB correction can be done only ONCE in your lifetime.

POA (Proof of Address)

For address correction

• Any one of the 45 documents mentioned in the list of supporting documents. • Photo of the resident is required wherever it is asked for in the list. Photo must have cross sign and seal.

• Whichever document has expiry date, should not have expired. • Name of the applicant mentioned in the document and in the Aadhaar should be same.

• Certificate or letter issued by the authority must and should have exact information. The validity of this certificate is 3 months from the date of issue.
