Delay in issuing Death Certificate to widow of ex-serviceman

Sir,

Late K.K. Chengappa, an ex-serviceman from Raghavendranagar, Mysuru, died at the District Hospital on Aug.24, 2020. It is needless to mention that widows of ex-servicemen require minimum 10 original Death Certificates of their husbands with correct initials and name to claim defence family pension and for various other purposes. 

The deceased ex-serviceman’s son Nikil Nachappa claims that the Hospital issued only one Certificate with the initial and name of deceased as ‘KKI Chengappa’ instead of ‘KK Chengappa’ on Sept.14, 2020. 

Nikil Nachappa informs our Ex-servicemen Association that despite requesting to correct the initials of his father in the Death Certificate and issue 10 original Death Certificates,  the same has not been done thus far and the family members are being made to run from pillar to post without giving any convincing reasons. 

Hence, on behalf of the bereaved family, I request the  Medical Officer and authorities concerned to take suitable action to issue the correct and required number of Death Certificates at the earliest so that the deceased ex-serviceman’s widow could claim military family pension.
